Every congregation needs a simple way for members and visitors to raise their hand for a ministry, or just update their contact info, without cornering someone after the service. This template gives you a ready-to-use "Church Connect" card sized four-to-a-page for easy printing and cutting, so you can drop a stack in the pews or bulletins and let people respond in the moment — cards go right in the offering plate for follow-up.

The card is intended to help fill volunteer needs within the church- swapping out different volunteer opportunities weekly. It includes space for name, phone, and email fields so that ministry leaders can follow up.  It also allows people to note a change in contact information.

The sample shown includes a nursery ministry checkbox as an example of how to phrase a ministry-specific ask — easy to swap out for whatever ministries or interest areas your congregation wants to highlight.
